Plus, the coverage is portable, staying with you no matter where your career takes you—as long as the group policy remains in effect, you pay your premiums when due, and you maintain your ACS membership.

  • Whether you’re applying for a single insurance policy or several, your application doesn’t involve any risk, cost, or obligation.
  • Once your coverage is issued, you decide which to keep, reduce, or refuse.
  • There is only ONE application form to complete, even if you choose to apply in more than one plan!

We offer the following Group Insurance Plans: Term Life Insurance, 10-Year Level Term Life Insurance, 20-Year Level Term Life Insurance, High-Limit Accidental Death & Dismemberment Insurance, Disability Income Insurance, and Hospital Indemnity Insurance. Please refer to the "plan description" links on the left hand side of this page for more information including features, costs, eligibility, renewability, limitations and exclusions for each of these plans.
How Do You Wish To Apply?
Apply Online: Complete your application form online and submit it electronically.

Once you begin, you will be able to save a partially completed form and return back to it up to 10 days later. In order to do so, select 'Save For Later' anywhere in the application. Then when you return, click 'Retrieve Saved Data' and enter your saved credentials.

Once we receive your electronic application, you will be contacted by phone for your medical history. You will be advised if a medical exam, blood and urine samples, and / or additional health or financial information will be required in order to process your application. Any potential exams or specimen collection will be conducted at a time and place convenient to you and at our expense.
